Apply for Temporary Provisional Licensure

Before proceeding to fill out the request form below, it's important to confirm you have created an account on EPSB's website and your hiring district has initiated the CA-TP. You can do so by clicking HERE. Once you've completed this step, please proceed with the appropriate CA-TP Request form linked below. We offer two options tailored to your convenience: one for initial CA-TP requests and one for candidates seeking to renew their Temporary Provisional License.

Initial CA-TP Request

Prospective candidates enrolled in the MAT or Proficiency programs are invited to apply for EPP recommendation for their initial provisional license.

Renew CA-TP Request

Apply for renewal of an existing provisional license. The application for renewal of a provisional license is available biannually (June and December).

You can do so by clicking HERE. Once you've completed this step, please proceed with the CA-1 Request form linked below. We offer two options tailored to your convenience: one for current graduates with access to their University of the Cumberlands email account, and another for past graduates who no longer have access to their university email.
